    **NEW** Administrative Cost

    A one time non-refundable administrative fee of $50 is be paid at the time you submit your form which covers the cost of ALL printed materials used for the deliverance questionnaire, processing of documentation (scribing), video conference, and the development and documentation of the prayer strategy for the administration of deliverance.

    Please submit your $50 administrative fee payment here.

    IMPORTANT: Your Deliverance Administration Form must be submitted by the 1st Tuesday of the month’s session you desire to participate in (see Events page for deliverance administration session dates). If your form is submitted after the 1st Tuesday of that month, you will be scheduled for the following month’s session instead.
